Equipment Handout - PW & PREP Tackle Divisions 

Coach will contact you and let you know what team your child is on and what time equipment handout will be. 

A parent or legal guardian MUST be present to sign paperwork.  Birth Certificate MUST be on file. 

If NO Parent and NO birth certificate, player WILL NOT get equipment.

Please allow an Hour and a Half (1 1/2 hrs) for equipment handout.  

Payment must be paid in full in order to get equipment.

REMINDER - ORIGINAL birth certificate must be on file and 

SIGNED ORANGE BOWL FORM MUST BE TURNED IN - if these form are not on file NO equipment will be issued.
